Registration
Deadline: July 9th at noon Teams are being formed now for
give your names to any PCS or PHO officials or
register on-line or through e-mail or phone. This form must be completed
accompanied by the registration fee and handed over
to PCS /PHO Sports Directors, or be mailed to P.C.S.
or PHO for teams by July 9th at noon. For
individual sports by July 16th 1:00pm or e-mail your
Participation interest to P.C. S. at
SportsFestival@PCSChicago.org or
www.PCSChicago.org

Teams are being formed for: Kabaddi,
Soccer, Volleyball, Basketball, Track/Field, Shot-put,
Self Defense.

All fields are required for on-line
registration.

PCS will
hold all legal copy/reproducing rights to any/all
material related to "Punjabi Community Sports Festival
2006" including but not limited to Photos,
Video, printing names of participants.

Team: Entry
Fee $25 per team/Sport. Out of state team (Team
sports only) registration is free.

Individual Sports: ($2.00 per person

One participant can play only in one team of same
sport.

Participants are responsible for any personal
injury due to participation in any game.

Any objection or protest must be in
writing accompanied by $25 fee before half time (Fee
is refundable only if the protest is right) to be
reviewed by PCS Sports Rules and
Regulations Committee. Referee's decision will
be final.

Participants
under influence will not be allowed to compete. Breathalyzer test may be given if needed.

PCS has the
right to ask the proof of age for any age-restricted
competition.
